Please present your official LabCorp lab request form. Make sure the date and time are added. There may be a $10.00 draw fee.
Please retain your receipt from your lab visit to submit to your insurance company.
Release of Medical Records:
Please print the form, fill it out, and fax or mail to the facility where the records are located. Dr. McNaughton will tell you what to write in the “To Release” section.

Communication
If you ever have a question about your treatment, please contact Dr. McNaughton. You may call or email and she will get back to you as soon as possible.
Late arrivals
Visits will take place during the scheduled time. If a patient arrives late, the visit may or may not still occur, but will end at the scheduled time.
Cancellation
Patients who cancel their appointment less than 24 hours in advance may be charged half the cost of the visit. Cancellation one hour or less before an appointment will result in a charge equal to the full cost of the visit.
No show
Patients who do not arrive for their appointment or who cancel after the start of the appointment time will be charged the full cost of the visit.
